1. What is Form V?
Form V is filed pursuant to Order 1, Schedule 1B of the Industrial Employment (standing orders ) Central Rules, 1946. It contains the format of the Service Card. As per the rule, every industrial establishment shall maintain a service card in respect of each workman in the form appended to these orders, wherein particulars of that workman shall be recorded with the knowledge of that workman and duly attested by an officer authorized on this behalf together with the date. Every workman shall be entitled to a service certificate, specifying the nature of work (designation) and the period of employment (indicating the days, months, years) at the time of discharge, termination, retirement or resignation from service.
2. Why is it important?
a. It is a statutory requirement;
b. It contains the details of the workman with the knowledge of that workman and duly attested by an officer authorized on this behalf together with the date.
