1. What is Form III Rule 16?

This form is related to the certificate of fitness of a worker which is related to the medical examination of a worker by the surgeon and the certifying surgeon shall issue his certificate in form III. As per rule 16 (3) of the motor vehicle rules, The certifying Surgeon shall, upon request by the Chief Inspector carry out such examination and furnish him with such report as he may indicate in respect of any undertaking or class of undertaking where- 

(a) cases of illness have occurred which it is reasonable to believe are due to the nature of work or other conditions of work prevailing therein. 

(b) adolescents are or are about to be, employed in any work which is likely to cause injury to their health.

2. Why is it Important?

This form is important because it is a medical examination certificate of the worker which reflects its medical condition, this form contains information related to the particulars of the person, despective marks or whether he is physically fit for the kind of work and this form has to be duly signed by the certifying surgeon.
