1. What is Form G?
Form G is filed pursuant to section 12 and rule 9 of the Maternity Benefit Central Rules. As per the provision any woman deprived of maternity benefit or medical bonus, or discharged or dismissed during or on account of her absence from work, may, within sixty days from the date on which order of such deprivation or discharge or dismissal is communicated to her, appeal to the competent authority. The appeal may be made in writing and either handed over personally or sent under a registered cover to the competent authority. The decision of that authority on such appeal shall be final
2. Why is it important?
It is a mandatory requirement under the law. This form prescribed the format in which an appeal against the employer can be filed by a woman employee if she is deprived of her benefits.
