1. What is Form D?
Form D is filed pursuant to rule 4(5) of the Maternity Benefit Central Rules. This form contains the format of the delivery or miscarriage certificate of a woman by a midwife to be provided to the employer by the woman employee. This is proof that the woman has suffered a miscarriage or delivered a stillborn child. The certificate shall be issued by a midwife.
2. Why is it important?
It is a mandatory requirement under the law. This form prescribed the standard format of delivery. Miscarriage certificate to be produced to prove the fact that the woman has suffered a miscarriage or has delivered a stillborn child at the time of delivery.
