1. What is Form C?

Form C is filed pursuant to rule 4(4) of the Maternity Benefit Central Rules. This form contains the format of the death certificate of a woman or child to be provided to the employer by the survivors/ nominee to receive the claim, in case the woman or her child dies at the time of delivery. The certificate shall be issued by a Medical Officer of a regional hospital or a dispensary or from a Registered Medical Practitioner or by the production of a certified extract from a death register maintained under the provisions of any law for the time being in force. 

2. Why is it important? 

It is a mandatory requirement under the law. This form prescribed the standard format of the death certificates to be produced to prove the fact that a woman or her child had died at the time of delivery.
